Takes jam's remarks into account when possible, add TODOs for the rest.

* bzrlib/transport/http/_urllib2_wrappers.py:
(ConnectionHandler): No more need to attempt caching, the cloning
process is enough.
(Opener): Use one password manager by instance.
(HTTPDefaultErrorHandler.http_error_default): Oops, provide right
parameters to InvalidHttpResponse.

* bzrlib/transport/http/_urllib.py:
(HttpTransport_urllib.__init__): An Opener is created for new
transports only, cloned transports reuse it.

* bzrlib/transport/http/__init__.py: 
Add a TODO.
